Tadalafil (Generic Cialis®)

Tadalafil is commonly prescribed to treat erectile dysfunction (ED) in men and is regarded as one of the most popular oral medication options for ED. It belongs to the phosphodiesterase type 5 (PDE5) inhibitors drug class. The four major PDE5 inhibitors are sildenafil (Viagra® and Revatio®), tadalafil (Cialis®), vardenafil (Levitra®) and avanafil (Stendra®). Medications in this class were originally developed as heart medicines, but found to be more effective in treating erectile dysfunction (ED). By decreasing vascular resistance and relaxing muscles, Tadalafil increases blood flow to the penis. Tadalafil comes in common strengths of 5 and 20mg, but generics that are manufactured in compounding pharmacies may be prescribed in slightly higher or lower doses. ||||The duration of action for the treatment of ED (up to 36 hours) appears to be longer than that of sildenafil and vardenafil. Because PDE5 inhibitors promote erection only in the presence of sexual stimulation, the longer duration of action allows for more spontaneity in sexual activity.

What should I mention to the doctor during consultation?

During your consultation you should mention if you have any problems with your heart, liver, kidneys, vision disorders, bleeding disorders, or penile deformities. What happens if I take too much?

Taking more than the recommended amount of sildenafil greatly increases your risk of side effects, including severe ones that can be dangerous, such as severe low blood pressure or an erection that won’t go away even after 4 or more hours (priapism). Do not take more than the amount prescribed to you by your doctor. If you have any severe side effects seek out emergency medical attention immediately.

Tadalafil is administered orally and is most commonly prescribed for the treatment of erectile dysfunction in men.

  • Sildenafil comes in typical doses of 25, 50, or 100 mg, but generics that are compounded in compounding pharmacies may make the drug in slightly higher or lower doses.
  • Ideally, you will take the prescribed dose 30- 60 minutes prior to intercourse, preferably on an empty stomach. The drug can last up to 5-8 hours if you need to take it earlier. The time that it takes to eliminate Viagra® may be prolonged in older people and people with liver or kidney disease and the drug may work longer in these people.

  • The most common side effects of sildenafil include headaches, facial flushing, stuffy nose, and upset stomach. Serious side effects that are rare include an erection that will not resolve even after 3–4 hours (priapism), hearing loss, which can be sudden, and vision loss in one or both eyes.

  • Take medicines called nitrates (such as nitroglycerin)
  • Use street drugs called “poppers” such as amyl nitrate or amyl nitrite, and butyl nitrate
  • Take any medicines called guanylate cyclase stimulators such as riociguat (Adempas)
  • Are allergic to sildenafil
  • Are advised to refrain from sexual intercourse because of cardiovascular risk factors
  • Have with severe liver impairment
  • Have kidney disease
  • Have very low blood pressure
  • Have had a recent heart attack or stroke
  • Have hereditary degenerative retinal disorders
